Research Notification

I agree to allow the data collected by ShugaTrak^® to be used for research purposes and to allow it to be shared, after identifying information has been removed, with qualified researchers who wish to conduct statistical analyses of these data. Further, I understand that the de-identified data may be combined with other data sets to increase the power of possible analyses. I also understand that researchers who are given access to the data will be asked to provide assurances that they will not attempt to re-identify any individuals but that there is a small risk that the data may be combined with other sources of information to make identification possible and that, in the unlikely case that an unauthorized and malicious person were to obtain such a combined data set, it may be possible to connect the data to a particular individual.
